If your entire appendix OR any part of your appendix is copied from another source, you need to give a note at after the data or information to cite both the original author and the copyright holder of the information.
Format
The format used for the copyright statement in a note is the same as the format for notes in tables and figures. You can see examples of the format used in copyright statements according to different sources in the "Pictures, Tables, Figures, & Copyright" LC guide.
